What is an Exemplar Document?
The exemplar showcases a notary's ability to perform electronic notarizations and is requested by state authorities as part of the application or renewal process for remote online notarization (RON) authorization.
Some states, like New York, require notaries to submit an exemplar for every platform they use, and the state must approve the exemplar before the notary can legally conduct remote online notarizations.
States that commonly require exemplar documents include Alaska, Michigan, Nevada, New Mexico, New York, Oregon, Texas, and Tennessee.
If you are in a State that requires an Exemplar document, you will see the Exemplar Document step in your profile.
Important: Not all states require you to submit this document. Stavvy makes this step available in the event that it's needed and the step cannot be skipped. Please make sure you are following the requirements for your State.
State-Provided Exemplar Document
If your state requires a specific document, please make sure you have it in PDF format before proceeding.
States that have a state-provided document include, but are not limited to, New York and Texas.
No State Provided Exemplar Document
If your state does not require a specific document, you can download our standard template by clicking the "click here" link.
How to Upload a New Document
- Log in to connect.stavvy.com
- Click your name in the upper right corner
- Click Settings
- Go to the Notary tab
- Go to the Notary Profile Exemplar Document section and click Start.
- If you need a template, one can be downloaded by clicking "click here".
- Click Upload file and locate your document on your computer.
- Requirement: Your file must be in PDF format.
- If you downloaded the Stavvy template, it will be located in your Downloads folder.
- Click Upload
- Click Next
Add Your Electronic Signature and Notary Seal
- The annotation editor screen will open.
- From the right side, click Signature and Seal. This will place the annotations in the middle of the page.

Click and drag each annotation to the appropriate locations. Note: If you are using a state-provided document, be sure to add any additional annotations that may be required.
- Click Mark ready to sign
- Click Next
- Click on the Signature and Seal annotations to apply them to the document
- Click Submit.
- Your Exemplar document has now been successfully uploaded. It will take a minute for the document to be finalized.
You'll know it worked when you see a "uploaded and notarized" version from today
- If the document needs to be sent to the State, click Download.
